Trouble Creating Custom Paper Size for PDF

i having trouble setting custom paper size.  'properties' of adobe pdf printer going 'printing perferences'.  'adobe printing settings' tab, clicking on 'add' adobe pdf page size.  naming page size , inputing desired width , height.  next, click on 'add/modify' button.  'add custom paper size' window closes has added it, not in select list.  doing wrong?
