i trying create form user being able check options such excellent, good, poor, etc.; i'd user able either hover on box or click on more information or ideas on criteria need make choice. best way this? secondly, there easy way put comment box @ end of each question or best have @ end of section?

(1) add first formatted text field , rating scale field. explain criteria choosing choices (excellent, good, poor...) in formatted text field. or

 

(2) add explanation in rating scale choices example, "excellent (choose field when meet criteria)"  yes, have brief explanation. or

 

(3) add tooltip (select rating scale field , enable "include text" in field properies panel) rating scale field, , include explanation there.
