Separate PDFs from one template form?

hi, when first meet our clients, send link form reused/submitted maybe 20 times. how can save these separate forms separate pdf's different titles , can open in windows document file? need able see which. thanks,

i'm not sure mean "...i can open in windows document file".

 

if form responses being submitted formscentral, use file > download selected response pdf form... menu item, accessible in view responses tab, save each submission separate pdf.

i using ubuntu desktop on lucid , keyboard changes british keys american keys, #/£, ~/| , "/@ around. in system->preferences->keyboard menu have set keyboard uk. when log via putty uk keys come out right. there other place more configuration required? /vfclistsguy actually settings should enough.. can try setxkbmap -query see current layout settings.
