Rating scale and text

i using rating scale , want include text field additional information.  ideally i'd scale text field right otherwise go underneath.  however, don't want have keep repeating scale.  ideas on whether possible or if there work around?

i guessing add additional information each of questions in ratings scale right?  assuming case, don't have work-around, think you'd have create rating scale field followed multi-line text field , copy , paste , modify each of questions.  let me know if didn't understand , feel free add feature idea our ideas page:

i using ubuntu desktop on lucid , keyboard changes british keys american keys, #/£, ~/| , "/@ around. in system->preferences->keyboard menu have set keyboard uk. when log via putty uk keys come out right. there other place more configuration required? /vfclistsguy actually settings should enough.. can try setxkbmap -query see current layout settings. how do I change the e-mail address for my merchant account
