Title in web form

i'm trying create new web form people subscribe newsletter.

i want form fields name , email can't seem work out how remove title , first name

any ideas?

you need first , last name on web forms.

you can combine them fullname - http://forums.adobe.com/docs/doc-2363

just delete title html form.
