"3+40 - 146: failed to load external url adobe.form.embed.min.js"

i trying embed form google website: sites.google.com/site/pjdsptostern, on pto dues tab.it giving me error message: 3+40 - 146: failed load external url adobe.form.embed.min.js. please help

for background, start taking @ http://forums.adobe.com/docs/doc-1991.

 

it looks me google sites has encoded form embedding inline script.  don't see script include of adobe.form.embed.min.js.  suspect tool used in google sites not allow including external script.

 

also, did type script in or did use 'embed' button on 'distribute' tab copy / paste embed script?  'formid' property in web page incorrect.  should 'mnonzbds0ua7ymhhaybukw' appears 'fmnonzbds0ua7ymhhaybukw' in web page -- note initial 'f' incorrect.

this script designed work web page supports both external , inline script.  web page frameworks not support external script because external script can violate design principles of particular framework.

i using ubuntu desktop on lucid , keyboard changes british keys american keys, #/£, ~/| , "/@ around. in system->preferences->keyboard menu have set keyboard uk. when log via putty uk keys come out right. there other place more configuration required? /vfclistsguy actually settings should enough.. can try setxkbmap -query see current layout settings.
