Is there a way to add a comment field with a rating scale?

i creating evaluation form , want capture ranking/rating comments. , sample question be:

5 = consistently outstanding

4 = above average

3 = satisfactory

2 = needs improve

1 = unsatisfactory

0 = no knowledge

develops , communicates strategic vision

this not support can add or vote on feature ideas:

if need add new idea click "create idea" under "actions" in top right.
