Table Of Contents Ordering Incorrectly

in design cs4, windows, network drive. 

 

we have several 180 page books composed of 15 documents.  if run table of contents 1 level...let's call centered title, runs perfectly, , when alphabetize entries checked, alphabetizes perfectly.  if add second level toc,  1 or 2 entries @ centered title level jump out of order.....cabin cabinetry leap in front of blu-ray player, instead of being should.  if take second level out, alphebetizes again.  same entry me.  coworkers similar publications have same problem, different centered titles.

 

the page numbers in toc fine.

 

this driving me crazy.

i using ubuntu desktop on lucid , keyboard changes british keys american keys, #/£, ~/| , "/@ around. in system->preferences->keyboard menu have set keyboard uk. when log via putty uk keys come out right. there other place more configuration required? /vfclistsguy actually settings should enough.. can try setxkbmap -query see current layout settings.
