dialoge box at page open comes several times

  have issue pop dialoge box.  created script that, @ page opening, ask if first letter being sent or 2nd letter.  field show or hide based on answer

this dialogue box occurs after other page open code looks see if there fields empty , hides templates if fields blank

 

the issue having pop box asks user choose first or second tick box - staff tick box , hit ok

but box pops 2nd time

 

then scroll down page tick box - first dialogue box occurs again

 

and yes, code @ page open there once

 

it not stop form working frustrating
