Searching for "File Attachment" button

i owning program named "adobe acrobat pro xi" , paid version. when want make pdf form using formscentral of it, not show "file attachment" button on "add item" menubar. have been searching button everywhere till now, never been succeeded. tell me button is? need respondents insert files only* in field or box whatever called instead of texts.

in general, of features within formscentral will work pdf forms create. however, file attachment can’t work in pdf forms. can find more information http://forums.adobe.com/docs/doc-2013.

 

now can use web form collect responses.

go distribute tab ->click email link button or copy link email, , send out email form filler. form filler open link in browser, able upload file attachment , submit.

i using ubuntu desktop on lucid , keyboard changes british keys american keys, #/£, ~/| , "/@ around. in system->preferences->keyboard menu have set keyboard uk. when log via putty uk keys come out right. there other place more configuration required? /vfclistsguy actually settings should enough.. can try setxkbmap -query see current layout settings.
