How to have a checkbox add formatted text on another page

i working replicate set of dense site survey forms use on 10" tablets... got of figured out.

i getting close having working survey form...

 

the form -is- final product, doesn't need uploaded.  filled in form printed , given client completed site survey.

 

got of issues solved...slowly

 

issue if selects options (checkbox's) , want put/trigger comment on "general notes summary" page.

 

i know how show/hide text based on checkbox using long javascript, considering have hundreds of possible checkboxes, want see if there easier way dynamically show/hide text on summary page , keep resemblance of formatting on summary page.

 

i thinking having multi-line field on summary page, , having script "add" comment field, there no way remove comment if box becomes "unchecked" unless there "clear & re-create" option. (messy think about)

 

i have read lot of methods on forums , web, , work small scale things, not when there hundreds of possible options, , "summary" page grow past single page.

 

most of comments of form:

 

" * item present risk structure , life safety , should repaired (yada yada line or so) <crlf> "

 

so summary page list of things need attention

 

hope has way scales , can create nicely formatted "summary page"

 

ps...i using acrobat_xi , have no javascript abilities, have been cutting/pasting snippets forums/web make things work , learning go.

i have figured out how print form wireless android tablet (share cloud printer, or wifi printer, works great)
