How do I get a radio button acting/check box looking "yes,no" set of checkboxes in InDesign CS6?

i'm able radio buttons or check boxes, not desired "pick one" yes/no checkbox. have found lots of general info not specific answer. help.

if i'm understanding question correctly, want function of radio buttons (choose one) appearance of checkboxes (choose apply).

 

if case have 2 options:

 

• create buttons in id radio buttons , edit button state appearances appropriate, or

 

• create labels in id , make buttons in acrobat, there 6 visual options radio buttons.

i using ubuntu desktop on lucid , keyboard changes british keys american keys, #/£, ~/| , "/@ around. in system->preferences->keyboard menu have set keyboard uk. when log via putty uk keys come out right. there other place more configuration required? /vfclistsguy actually settings should enough.. can try setxkbmap -query see current layout settings. how do I change the e-mail address for my merchant account
